Desk Review Audit Requirements for Golf Courses
Starting this year, all desk review audit forms must be submitted electronically at: www.ipmcouncilcanada.org You will need a user name and password to upload your files. Go to the website and click on the Members tab for instructions to obtain your on-line account.
2011 Desk Review Audits are due by January 31, 2012
ALL of the following forms must be included in your 2011 Desk Review Audit submission. You may complete all the forms electronically on the IPM Council of Canada website, using your user name and password. The forms are available in the IPM Golf Bag section of the interactive website. If you have been maintaining this information on paper forms, you may either transfer the information onto the fillable PDF forms or scan the signed paper forms for uploading to the website. To view and print, click on the following forms:
- Scouting Form - Monitoring is required a minimum of once per week throughout the growing season.
- Pest Control Product Application Form - Record of product(s) used, amount of product(s) and active ingredient(s) applied, volume of water used, location and reason for use, climatic conditions, irrigation (if any), and follow up for every pest control product application.
- Equipment Calibration Form - Field Boom Sprayer - Regular calibration of application equipment is required to ensure the correct amount of product is applied. Complete all data from one of the calibrations performed this season plus any additional calibrations performed due to equipment problems noted during usage. Maintain proof of calibration (worksheets or receipts from an external calibration service) from all calibrations performed this season for inspection during on-site audit.
- Equipment Calibration Form - Handheld/Backpack Sprayer - Regular calibration of application equipment is required to ensure the correct amount of product is applied. Complete all data from one of the calibrations performed this season plus any additional calibrations performed due to equipment problems noted during usage. Maintain proof of calibration (worksheets or receipts from an external calibration service) from all calibrations performed this season for inspection during on-site audit.
- Annual Report – Pest Control Product Usage - Annual Report, as required by Ontario Regulation 63/09, summarizes total pesticide usage for the year. The annual report will also include a map of the golf course showing where the Class 9 pesticides have been applied, contact information and must be signed by both the IPM Certified Agent and an owner representative.
- Training Documentation - All staff responsible for pest control product applications and scouting/monitoring should receive annual training. If only the IPM Agent is involved in IPM activities, list the continuing education received. This may be a repeat of information submitted to the University of Guelph, Ridgetown Campus for CEC requirements, but the environmental auditor does not have access to this information. Also required is submission of the Attendance Register. All staff receiving training must sign the register as proof of attendance.
Please note: The Hot Spot Management Record is no longer required for the Desk Review Audit. Most of the information is captured in your Scouting Forms and the IPM Advisory Committee deemed this document redundant.
Once all forms have been uploaded into the My IPM section of the www.ipmcouncilcanada.org website, your 2011 Desk Review Audit is ready for audit. Contact the IPM Accreditation Program at ipmap@ridgetownc.uoguelph.ca with your user name and password to let them know your 2011 Desk Review Audit is ready for audit. All the documents listed above must be uploaded before an audit will be performed.

- 2011 Continuing Education Credits (if required for 2011)
