Desk Review Audit Requirements for Golf Courses
You must complete and submit the following forms to complete the Desk Review Audit process. To view and print, click on the following forms:
- Scouting Form - monitoring required a minimum of once per week throughout the growing season. Four random weeks will be requested by the administrator at the end of the season.
- Pest Control Product Application Form - record of product(s) used, amount of product(s) and active ingredient(s) applied, volume of water used, location and reason for use, climatic conditions, irrigation (if any), and follow up for all pest control product applications.
- Equipment Calibration Form - Field Boom Sprayer - no less than 3 calibrations must be completed on each piece of application equipment
- Equipment Calibration Form - Handheld/Backpack Sprayer - no less than 3 calibrations must be completed on each piece of application equipment
- Hot Spot Management Record - record of actions taken to manage a problem area throughout the year and future management plans.
- Annual Report – Pest Control Product Usage - Annual report, as required by Ontario Regulation 63/09, summarizing total pesticide usage.
